This was most definately mechanical failure. The nut behind the wheel.

I was speaking to a guy at work who had the recovery point on his pajero fail last week. I think he has a NM Pajero, it had the recovery point which bolts on the back.
They were out near Geelong when he got stuck in a boghole in a quarry. His mate in a 2 week old Navara said he would snatch him out. The Paj driver said just a gentle pull was all that was needed but the Navara driver insisted on a flat out wheel spinning snatch.
The bolts on the recovery point sheared off and it ended up in the back of the Navara, making a nice big dent.

They then attached the snatch strap to the towbar and gently pulled him out!
      thanks,

There must be more to this story. I don't doubt that the recovery point came off, but to shear four high tensile bolts which are over 10mm in diameter? With a snatch strap? No.

Something else had previously happened to stress the bolts. My guess would be somebody lubricated the threads and then over-tightened them.

If those bolts were properly installed the loop would fail before the bolts shear.

Correct me if I'm wrong, but the NM doesn't have a tow point at the rear of the car. They have two tie-down points for vehicle transportation.
It's more likely to be an NL, (or any Gen 2) and then I wonder if it had a towbar fitted that used the tow point mounts. If longer bolts weren't put in, that would explain why the tow point could fail on a Gen 2 Pajero.
